The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any cremation/funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the cremation/funeral goods and services you selected.
This list is not meant to show all of our available services. A full general price list is available at any of our 9 locations.
REGISTRATION and MEMBERSHIP
Completion of the simple Registration and Authorization form enables prompt fulfillment of the member's instructions at the time of death. A membership card is issued and a permanent file is maintained at the Society office. This fee defrays the cost of setting up and maintaining your records. It is not refundable nor an offset to final service cost.

| The following are current fees to be charged for service rendered at the time of death. Unfortunately because of inflation, these prices will increase during future years. However, you do have the opportunity to pay the cost at today's prices by depositing the necessary funds in the Cremation Society of Illinois' master Trust plan. Thus, future services are guaranteed and fully paid for the rest of your life. Information concerning the trust plan may be obtained from our Society office. |
| DIRECT CREMATION* |
| This charge includes: transfer of the remains to our facility, necessary services of staff, care of body, local transportation to the crematory, and cremation service charge. This charge does not include visitation, rites or ceremonies prior to cremation of the remains. (An additional charge of $1.50 per mile driven in excess of 30 miles from any of our locations will be added.) If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The minimum container we provide is fiberboard. |

| OTHER PROFESSIONAL SERVICES AND MERCHANDISE |

| BASIC SERVICES OF STAFF | $605.00 |
| Our charge for basic services includes, but is not limited to, the services of our staff to respond to initial requests for service; the services of our staff for an arrangement conference with the family or responsible party; arrangement of the cremation services and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices. Included in this billing, secretarial and administrative costs, maintenance and utility expenses, equipment and inventory expenses, insurance expenses and similar overhead expenses. In the event that it is necessary to use the services of an outside funeral director, their fees may be billed seperately. This fee for our basic services and overhead will be added to the total cost of the cremation/funeral arrangements you selected. (This fee is already included in our charges for direct cremations.) |

| Removal of the deceased from the place of death and other transportation needs within a 30-mile radius of any of our locations (Additional mileage of $1.50 per mile, one way). This fee is already included in our charges for direct cremation. |

| CREMATION and REPOSITORY FACILITY | $360.00 |
| Three to five days, or longer, will lapse from the time of death until cremation is completed. This time span necessitates refrigeration, thus eliminating the need for embalming. This fee is already included in our charges for direct cremation. |

| EMBALMING | $495.00 |
| Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you have certain cremation/funeral services, such as a cremation/funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. |
